Are you or a loved one managing multiple health conditions? Arbor Health's Chronic Care Management (CCM) program might be just what you need.
The CCM program is designed for Medicare and Medicaid patients with two or more chronic conditions like diabetes, hypertension or hypothyroidism. Through this program, patients get direct access to a dedicated nurse who helps manage their health care needs.
"I like to think of it as a direct nurse line for your health through your primary care doctor," explains Kelli Cotton, RN, a CCM nurse with Arbor Health.
As part of the program, your CCM nurse will:
- Contact you by telephone at least once a month.
- Help schedule doctor's appointments and labs.
- Answer questions about your chronic conditions and medications.
- Create an individualized nursing care plan.
How CCM Helps Patients
"I think that one of the greatest benefits is you get a direct line to a nurse," says Cotton, explaining that the nurses in the clinics are busy rooming patients and helping the physicians, doing the tasks of their specific jobs. Because of that, it might take them a little longer to return telephone calls.
However, chronic care nurses do not have those extra duties. "I think that's one of the biggest benefits," she says.
Cotton reports that patients particularly appreciate having someone to explain medical information in everyday language and provide ongoing support for lifestyle changes.
"I think knowing that you're not alone in your chronic conditions and with questions that you have gives people peace of mind," says Cotton. "They've talked to a health professional and they have an answer."
